Submit Documentary Requirements

All in PDF format please including pictures and documents.


To all Applicants, please email me at luel_yukoya@chonghua.com.ph so that I can create a personal link for you to submit all documentary requirements.


For New Applications, please start with #1 : Application Letter and wait for our reply and instructions to proceed with the remaining requirements. 


For Renewals, please complete everything at your convenient time, preferably before the expiry of your present accreditation.


Please submit documents as complete as possible. As the survey would be hybrid, proper photos and even video tours may be of help.

If you need to update documents or add more information or correct some mistakes, please resubmit the entire section as we shall only consider the latest submission to avoid opening multiple documents and cause confusion.

Once you have completed all documentary requirements, please email me at luel_yukoya@chonghua.com.ph so that we can start reviewing and schedule a hybrid visit.
